	3 – coloured photographs passport size
	Valid identification
	A letter of introduction from the applicants employer or,

	Letter of Introduction from Ward Executive Secretary (for those who are not employed) affixed with passport size photograph or an introduction from a recognized referee maintaining Uhuru card or any account with Tanzania Postal Bank.
